چکیده

39 Interactions between drugs and nutrients often occur and can have a detrimental impact on patient outcome (1). The risk of pharmacologic-nutritional interactions continues to grow as specialized nutrition support (i.e., enteral and parenteral nutrition) is initiated earlier and the use of multiple medications increases. Recognition of clinically significant interactions can assist with prevention or early treatment of adverse consequences arising from drug-nutrient interactions. This article will focus on basic knowledge needed for identification and appropriate management of drug-nutrient interactions in patients requiring enteral nutrition (EN) and parenteral nutrition (PN). Drug-nutrient interactions will be divided into four categories based upon their mechanisms: 1) ex vivo biopharmaceutical inactivations; 2) interactions a ffecting absorption; 3) interactions affecting systemic/physiologic dispositions; and 4) interactions a ffecting elimination/clearance (2). Each category will be discussed separately and examples will be given to illustrate the use of alternative methods to assure safe drug administration.

Nutrition considerations in traumatic brain injury.

The provision of adequate nutrition support for patients with traumatic brain injury (TBI) has been a clinical challenge for decades. The primary and secondary injuries create unique metabolic derangements along with accompanying issues such as optimal timing and route of nutrition, appropriate fluid and electrolytes, drug administration, rehabilitation, and dysphagia. Enteral nutrition is clea...

Comparison of the Effects of Intralipid 10% and 20% on Peripheral Catheter Ablation in a Pediatric Intensive Care Unit

Background: Energy deficit is a common and serious problem in pediatric intensive care units. Parenteral nutrition, either alone or in combination with enteral nutrition, can improve nutrient delivery in critically ill patients by preventing or correcting the energy deficit and improving the outcomes. Intralipid 10% and 20% are lipid emulsions, widely used in parenteral nutrition. Despite sever...

Nutrition in pancreatic diseases.

The pancreas plays a major role in nutrient digestion. Therefore, in both acute and chronic pancreatitis, exocrine and endocrine pancreatic insufficiency can develop, impairing digestive and absorptive processes. These changes can lead to malnutrition over time. In parallel to these changes, decreased caloric intake and increased metabolic activity are often present. Nutritional deficiencies ne...

Advantages of enteral nutrition over parenteral nutrition.

It is a strong and commonly held belief among nutrition clinicians that enteral nutrition is preferable to parenteral nutrition. We provide a narrative review of more recent studies and technical reviews comparing enteral nutrition with parenteral nutrition. Despite significant weaknesses in the existing data, current literature continues to support the use of enteral nutrition in patients requ...

American Gastroenterological Association Medical Position Statement: guidelines for the use of enteral nutrition.

T he need to avoid prolonged starvation in patients is well recognized. More recently, it has been realized that without intraluminal fuels, intestinal integrity may deteriorate and allow translocation of gut bacteria. One means to combat nutrient deprivation and simultaneously to keep the local defense barrier of the intestine intact is tube feeding, i.e., enteral nutrition. This technique has...
